Okay, about midway through Stage 3, there's a part where there are four statues and a cauldron (?). If the statues are blown up, the cauldron releases some items. Typically it's just a weapon powerup and a bomb. Sometimes though it releases an item that restores half a life bar.
I messed around with savestates a bit, trying to destroy the statues in different order, waiting different amounts of time to destroy them, etc etc... I tried everything I could think of, and I couldn't find any reliable method to get that life-up to appear. As you know, that life-up is important since when you have a full bar it adds 2000 to the chain. Stupid combo-reliant scoring system... bah, anyway. Do you know what the deal with this is?
OK, you must defeat all enemies on the screen (also the two birds that fly from right), after to have kill the birds you must concentrate the spirit on the first statue on the left (you must make it before that the screen moves towards the high), the spirit must be activated on the statue without to touch other still alive enemies

(sorry for my english)
Hmm... I coulda sworn I got the lifeup to appear once when the birds were still on the screen. And I've gotten it by destroying the statue at the top-right, instead of the bottom-left. I'll try this again though.
Could it be possible that you have to destroy a statue when the "combo meter" (the skulls) are flashing gold? That would kinda make sense.
